Entering an instance in a party will prompt other party members to join. Party members will automatically attempt to join the same map as the first party member who enters. So, the normal guild hall instance only holds 5 players. Once one more person enters, it asks everyone to volunteer for an instance that can hold 10 people. Once an 11th person joins, it asks you to volunteer again for an instance that can hold 15 people. And so on.
If you're both in the same open world map, then you should be able to join each other's instances (unless they're full). If you're in story instances (including the first one when you're still level 1), in dungeons, in WvW or PvP, and possibly a few other situations I forgot, then you cannot join each other that way.
If you can see your friend, click on their character, then right click their character portrait and choose Group -> Join Group from the menu. You can also right click their portrait and choose Additional Commands -> Add to Friends list if you want to better see when they come on and offline.
